TEAM CARE MEDICINE – Course Syllabus

The purpose of the curriculum is help the provider rapidly develop a highly functional and pleasurable practice-based care team that the provider leads. The care team is the most important component of the Patient-Centered Medical Home. The curriculum will start the team building and system re-design that will yield the benefits of improved documentation and quality of care, improved productivity and finances, and improved patient and clinical team satisfaction.

LIBERATING THE FAMILY PHYSICIAN - Demonstration, Training & Transition:

This 2-Disc DVD Set is an audio-visual teaching aid led by Dr. Peter Anderson personally, including an introduction to Family Team Care (TEAM CARE), a short patient interview, expected results in transitioning to TEAM CARE and the 11 steps in the transition process from traditional care to TEAM CARE. DVD #2 presents 4 general rules of data gathering and a detailed breakdown of the patient interview, dividing it into 5 segments. Also on DVD #2 there is a full patient interview by an assistant, a session with Dr. Anderson and a MA, and comments by RNs.

HANDBOOK OF TEAM CARE FOR 21ST CENTURY FAMILY MEDICINE:

This 288-page manual describes the Family Team Care (TEAM CARE) Approach and is a working companion for motivated and well-trained RN’s and LPN’s. It is not intended to be a comprehensive treatise on diseases and their symptoms—but rather, to assist the doctor’s staff to perform the part of the patient’s visit that does not require a physician’s clinical expertise. The physician’s focus can then be directed toward the tasks for which he or she has been specifically trained: Treating the patient.

5. Optional Content Licensing:

Participants have the option to license the program contents, particularly the symptom questions and suggestions from Dr. Anderson’s manual for incorporation into your chosen EMR system. This allows the EMR to both enable and document the patient interview process to smoothly accomplish collection of the patient’s data prior to seeing the Doctor. Custom development assistance is available upon request.

6. Annual Support:

Support includes a 24 hour call back guarantee to answer questions and help you solve issues. In addition, Team Care Medicine will conduct two 1 hour post go-live conference calls with Dr. Anderson and the Team Care nurses. The support program may also include future access to additional material upon release, including our database of frequently asked questions, best practices, case studies and success stories from other implementations. Additionally, Team Care Medicine clients will be offered discounted access to nurse training modules and additional medical practice disciplines such as Pediatrics, OB/GYN and others will be made available as they are completed.
